Tie-breaker goes to healer or tank. Preferably healer.
It depends on what role I am playing. If I am a DPS, I will normally give it to the Healer. If I am a healer, I will normally give it to the Tank.
There are exceptions though. For example, if I or someone else is new to the dungeon and asks for help, I may give my commendation to whoever was the most helpful and understanding. I may give it to a new player who has done well on their first try also. I will not give it to a tank who ignores advice or is just plain bad though.

I give it to whoever performs their job well, and if everyone does, I give it to whoever was more sociable.
I give my commendations to the person that was more enjoyable to run with. Most of the time (not all), I get in groups that know exactly what they're doing, how to play their job, and everyone makes the dungeon run a smooth experience. Personally, I'm not gonna give the tank the com just because he's the tank. Same for the healer. Whoever made the dungeon most enjoyable (idle chit chat, jokes, just an all around awesome player to be around, gets my vote.
I like to give commendations to players new to the instance, if they pick up quickly and act well, they deserve it.
Next I do try to give to a DPS, if they were somehow memorable and essentially weren't acting badly, otherwise I'll give to healer or tank.
I won't give commendations to people that rip on other people for a bad run. I mean it happens. I won't hold it against people for expressing disappointment, or leaving when they said they were on a time limit, just don't be mean to others and making the run unpleasant. If you're constructive and not being accusing, that's okay. If they don't listen, they likely won't get the commend... just little things. It often goes with how the dungeon plays out.
I also like sociable people, and I will chat if others do, but if no one's chatting, I'll be quiet too.
Probably a sad thing, but I find myself giving the commendation to whoever's left. Maybe I'm just doing it wrong and not quick enough to make the decision.
That being said, I generally just go with the one who impressed me the most, naturally. Be it tank, healer, or DPS. Probably the MOST impressive thing for me is to see who holds up when things go wrong.

When I ran as a tank, it was so random if I'd receive commendations, i'd see flawless runs,(good heals, but more often teamwork to mitigate too much damage) not a single death, and i'd be doing decent pulls, nothing too slow. no commendations.
It seems more often than not anymore, the tank has to go above and beyond to get a commendation, but if i run as a healer, even low level and i'm didn't do that well, 3 commendations. frequently.
Regardless of skill i was outputting, i got them all, that seems odd to me.
and of course my DPS, rarely gets them outside of an 8-man, when I LB3 the final boss for no reason and kill him. Like he wouldn't die anyways in a few seconds, but that sure gets you a few commendations for some reason.
Mostly to tankers or healers ... in rare cases I give a commendation to DPS, when they don't: die, pull mobs before tank, steal agro from tank's non-target mob, curse other players.
