What do players even do during ERP ?

Maximum cringe of course.

Specially when old men tries to act all girly.

If you have to ask this you're probably too young for it to be legal for anyone but your parents to answer it.

From my understanding is that they use custom mods to do unwholesome things.

Now I understand why I always need a shower after being in Limsa...:p:D

It can happen anywhere. It might happen when you're sitting on a bench in one of the main cities, and the other will rudely sit on the same spot as your character to make it look like you're on their lap. It might also happen when you're /beesknees at a nightclub, and you get smacked with a full, novella-tier paragraph about being hit on and touched by someone dancing close by. Either way once the pleasantries are exchanged, and one of the two parties has "explored the body" of the other quite indiscreetly to turn them on (foreplay as it's called), the horses are off.

Long, lengthy, overly described stanzas are spammed back and forth - or, that's what I've seen at least, there's probably more scuffed writing out there as well - while the characters' gears are off for full immersion. And that's all of it, really. Hours can pass by during the course of this roleplay, assuming both parties are really into it lol. I guess some people use mods like Anamnesis to manipulate poses and add nude textures; find yourself the Endless Nights Events Discord server, check screenshots channels in it or any other Discords linked in there, and you'll see floods of photos like that.

I don't think many people care about how the other person looks, when reading their smut.
I guess it's not much different than reading a steamy novel - it does not matter how the author looks, right? It's all about the novel's characters, and the same might be true for eRP.

Accurate. Have had friends in the past who engage in the nightlife scene and they really don't care about the appearance of the other person. In that space, you're viewed as your character and they're after the interaction with your character, not your real life self.

One in particular comes to mind, said person had a harem at one point consisting of like...7 people and he was fully aware that some were men. As long as they kept it to their female persona and didn't try to take it to a real life point, he didn't mind. I personally didn't understand it but hey...if it works it works. Wasn't my gig in the first place.

If someone is roleplaying normally and the story happens to lead into an erotic scenario, do you start ERPing or is it considered normal RP?

One time my friend went to a club and paid one of the workers to play online Battleship with her. She streamed the Battleship match to me and some other friends on Discord. So obviously that's what people are doing at these clubs.
