I think I figured out the process how the same people are winning every tournament but not 100% sure...
Player A - the winner
Player B - the one who gets used
They start the tourney as soon as it opens...literally.
A has a one star deck. B has a max star deck. A beats B. A in first and B in second in the standings now. A continues to beat B as long as the points B gets for a loss keep them in second, if not A takes a loss to B to give B a boost back into second. Rinse,repeat until done. This means A(in first) is always beating B(second-ideally) with a one star deck vs a max star deck. So the points A is getting cannot be matched anymore by anyone since no one else can beat the ones in first or second for the rest of the tourney? I don't know exactly how the scoring works in regards to the standings so I'm wondering is this right?

Pretty much. Square doesn't want us to farm NPCs and tried to "encourage" us to play real people, hence why the points earned for beating a player is higher than beating an NPC. All this did, however, is push people to either cheat, or cheat even more by rigging games with friends or their alts. Somehow, people with ZERO wins are always in the top 20 because losing to certain players nets them more points than simply beating them.

It comes down to how they treat 1st place, 2nd place, 3rd place etc as though their meanings were static even though they aren't. During the first game of the tournament, sure the players are in first and second place, but they're also in last and second-to-last place. SE is ignoring that part and looking only at "1st place" "2nd place".
Giving a point boost when playing real players might make some sense (even if it is susceptible to win-trading), but giving bonuses based on your opponent's rank position needs to take into account where that position falls amongst all the players in the tournament. The opening few games should treat both players as being in the middle of the pack. (Which they actually are, it's just a smaller pack at that point.) Only once the tournament is far enough along for being 1st or 2nd or whatever to actually mean something should beating a 1st ranked or 2nd rank player give you extra bonus points.
(That still wouldn't eliminate the win-trading, but at least it would prevent the position lockout the tournaments get with the current rules.)

Eliminate bonus points based on the ranking of your opponent or the strength of their deck. Replace MGP prizes for top 3 places with additional Platinum/Gold packs. MGP rewards are nice, but I'm tired of seeing filth who already have all 4 tournament cards continue to exploit the top spots because SE rewards them excessively for it.

Have they still not fixed this I mean seriously at this point what is the point in doing the tournaments and not getting a single platinum pack which will most likely just be Ultima again and again. Ugh SE just go home your drunk.

Triple Triad is just about frustrations right now anyway, tournaments for cheaters, low drop rate on the new Heavensward cards, unfair rule like random on a lot of npc's, lots of fun !

Never played a tournament and never will till they fix it, but it sounds like the best way they could go about fixing it is to have it match you up with random players and not let you play the same person twice. Also take away the points system and just have it calculated by the amount of wins, and if a lot of people tie because of that, give them all the first place reward. Another good idea would be to change the achievements related to it so you only need to participate to get achievement progress as 100 wins is practically impossible, this should draw some more new people into the tournament too.
They still need to fix Jumbo Cactpot too but the Triple Triad Tournament is even more broken and it's just ridiculous how long it's gone even though the fixes should be simple.
