I just dont understand how this is a personal problem do you see how many achievements we have on our list Ill never have them all and dont care too. This is form the wiki: After each dungeon, trial and guildhest, you can give 1 player in the party a commendation. I solo heal FL, coordinate everyone on the battlefield, save people from certain death repeatedly, make all of the right calls (for once) and get nothing.
I rock up to Ifrit EX in silver GARO armour, say nothing other than 'hi', get hit by every single mechanic - I'm a terrible Ninja - and walk away with all of the commendations simply because I looked cool.
Commendations really shouldn't be taken seriously. Meanwhile, instances are instances whether they're unsynced or not. Soloing an EX primal should count for the total just as much as facerolling Sastasha. -

Adding support for what others have written: If you are trying to keep a 1:1 ratio between instances and commendations, you're going to be sorely disappointed. The longer you play the bigger the gap between them will be. Because people generally throw them at the healer/tank if you continue to play mainly as a healer your commendations will likely end up exceeding your dungeon runs.
But as others have pointed out, commendations are not guaranteed. I've had a few times where I've made a stupid mistake, killed myself, missed most of the fight and still walked away with 2-3 commendations. Likewise I've had a few times where (I feel) I've saved the party from a wipe, clawed back from a near wipe and I've literally solo-healed to the end and held it all together long enough for us to get the win... and I walk away with nothing. That's just how it is.
If someone asks for it though, it's very unlikely I'll give it to them.
Also just like to point out that the dungeons achievement says "Complete X instanced dungeon raids or trials", nothing about unsynced. If they were to ignore unsynced like you suggest would need to also adjust the title of all those achievements to reflect that.
Also, I assume you don't PVP? Although people generally leave so fast commendations probably aren't given all that often, that's somewhere you can get commendations and it won't increase your dungeon instances as they don't count towards that. Basically what I'm saying is if you're sure you want to try for 1:1 you'll have to never do PVP. -
