SC>Infuriate>w/e shouldn't be possible because you shouldn't be building 5 Wrath before the adds pop. If you are, for whatever reason, you're doing it under the effect of Unchained and in that case you already used your Infuriate.
The best start is: pre-build HS on boss -> Defiance -> Infuriate -> SS add #1 -> BB add #2 -> Veng/Thrill -> SC -> Defiance off -> OP -> apply SE to Daughter -> BB -> SE -> etc etc
400+ basically requires near max STR and an i130-i135 wep (easier with i135). Sometimes mechanics can get in the way and murder your DPS, but that's rare since most mechanics have you hitting the boss at the same time anyway.
You can also completely cheese your DPS and stack adds but I wouldn't recommend that unless you're in for a lot of learning.

There's a lot of factors to account for, like how short the encounter is, how much uptime you get on Fracture, etc. etc. T10 is kinda wonky to judge your DPS on.

Third one is from a pug. Second one could have been higher if I hadn't died. Still missing a couple accessories - I don't have the dreadwyrm slaying ring so I can't go 5 slaying and having the neck and earring would allow for a more optimized setup in terms of secondary stats. I pick up the second set of adds in second add phase so that's a bit of a DPS loss. I know a couple warrior who can go as high as 445-455 consistently.
T11 also isn't terribly difficult to do

It is possible to pull hate off the MT if you unload early enough in the fight against a MT that is stacking VIT instead of some STR. Ideally, I like to do SE, followed by a BB combos with STR pot up. I'd rip hate by the 2nd BB, if I crit either of them. Certainly on the 3rd.
Some people tell me that the DPS difference of just rotating SE/SP vs SE/BB is minor. I don't buy it. I see 2k crits w/ BB as opposed to 1.6k with SE. That extra BB potency adds up. -

3rd BB? Surely you're joking as you would need to at least SE/SP every 3rd combo to keep maim up. SE followed by 2 BBs is the highest dps you can hit, there is a 20 potency different between the full combo of BB and SE and another 20 potency difference between SE and SP so it goes.
630 BB, 610 SE, 590 SP. A combo takes around 7.5 seconds to complete without any SS so you're looking at 8 combos a minute. So it should go SE->BB->BB->SE->BB->BB->SE->BB in one minute. SE debuff falls off about a second or two before SE is reapplied. -
