A few 4 chest runs, no 5 chest run and We have gotten Darklight Caligae from left chest, and Eyepatch from Gold chest.

Heavy Darklight Flanchard is the gold chest in CC (different from your list)
Darklight Calligae is the middle left chest (going left to right, chest 2) (different from your list)
Heavy Darklight Gauntlets is the far right chest (matches your list)

If you trust 2chan, they also say Darklight Sollerets are from AV middle left (chest 2 going left to right, same as boots)
Also I just noticed that the original post was edited to reflect pop conditions of chests. I really disagree with the pop condition for the far left chest in AV. The OP states "kill all slugs in the first 2 poison rooms". I can provide proof via video here:

At 3:40, we are popping the porter at the first poison room with 2 slugs aggroed. They depop on the video clearly not dead. We then end up with chest 1 at the end.
I think the pop condition of chest 1 has to do with killing a single mob in poison room 1, which may happen to be a slug. But you certainly do not need to kill every slug in the instance.
