I had this issue Sunday (around 1-5am PDT), while playing on Durandal (JP Legacy) and Fenrir (JP). I also experienced it Monday on Migardsormr (NA). I somehow managed to play on and off at various times yesterday and this morning with no incidence of it on Migardsormr.
I'm in the SF Bay area on Comcast (or I guess it's supposed to be Xfinity now).
What types of routers?
I went from running an Asus RT-AC66U to my Asus RT-N66U (AC Technology clearly isn't ready) and things went back to normal. No lag, perfect gameplay and such.
Im still at work but im discouraged to hear that they problem still seems to be happening! Its not your hardware or routers. Trust me. We went thru the same thing in Beta 3. Its something on their end or between them and the ISPs. They never came back and reported exactly what they did to fix it, only that it has been resolved which it was.
I am also having this problem, frequent periodic drops to Receiving 0 (R0) and sometimes Sending 0 (S0). I am also in Northern VA on Verizon FIOS. I'm using an actiontec MI424WR rev.1 router. The Garuda fight (and any fight with insta-kill mechanics) are downright unplayable. I'm dying to common mobs out in the world because R0/S0 causes nothing to occur while I'm jamming on my keyboard keys. FPS runs perfectly normal and there are no issues with CPU or memory. Please let us know if you need more information, and PLEASE resolve this matter ASAP.

Im having same issue on the Masamune server. Funny thing is that it only happens for about 2-3hrs a day. Between 8-10pm GMT. Getting really annoying as this isn't even the busy time. I wish someone from game would at least come and let us know if its the game , server of us?
I'm using Windows 7, I've already configured my router to forward the proper ports. The 100Mbps bottleneck of the router is the problem at all. Thank you for trying, but I'm pretty much convinced this is between Verizon and SE at this point.
Also, this phenomenon is occurring 24 hours a day for me. Doesn't matter if it's peak or off-peak.
I have a newly distributed router from Verizon (since the old one died last month). There is no backup firmware on the router, so a revision/version number would be appreciated if you were able to get it to consistently work.

Quick update: Out of curiosity, I signed up for a VPN service to see if I could indeed trace the problem to in fact be between ISP and SE. The fact that I've yet to wait 10 seconds to dismount is a good sign so far. I'll check back in later tonight with another progress update. HOWEVER, I should not have to pay for a service just to make this game playable. SE needs to figure out what the issue between them and Verizon so my monthly fee isn't $7 higher than it needs to be.
I would suspect that SE is prioritizing packet traffic as it comes in to and out of their NA/EU Data Center out of desperation to remedy their capacity issues. (This is called "Traffic Shaping".) If you look at the Ping rates of all the NA/EU Servers you'll find a 100+ ms difference between the lower populated servers when compared to the higher populated servers. When servers are located in the same physical location on the same physical subnet they should have comparable Ping rates. Otherwise, if there would be a difference, the higher populated servers which are receiving more traffic should be the ones to have higher Ping rates.
For the fact that the higher populated servers have significantly lower Ping rates across the board shows that SE recently made some routing changes at their NA/EU Data Center to help alleviate congestion on the higher population servers, but at the cost of the lower population servers experiencing an increase in lag.
As a band-aid they are probably doing a combination of Packet Shaping, Congestion Control, and Traffic Control to deal with the Bandwidth Contention (oversubscription, or too many connections sharing the same limited network bandwidth) at their NA/EU Data Centers. It will probably remain this way until their infrastructure is upgraded in the coming days (or weeks).
Good god. i pray its SE's problem. Cuz making SE and verizon talking to each other is going to be harder than pulling teeth. Especially the fact that all ISP are pretty much monopolies. I only have fios in the area. So if verizon decides its not important then its never going to get fixed
So far about an hour over the VPN and everything has been a dream. Very little in the way of lag. I'll report back again in a few hours.

Well, I can get in... and stay in... but goodness healing instances is a pain in the neck.

Checking back in. Everything has been close to how the game "should" be performing. I don't know how else to contribute at this point other than saying, "SE please figure out why Verizon FIOS users of the mid-atlantic US (and the other areas suffering this) are experiencing this and fix it right away"! I really don't want to have to pay more money on top of my soon-to-come monthly fee to play!
I'm elated, but somewhat dismayed to see people having the same issue I am. I'm also in the Northern Virgina area with Verizon FIOS (50/25). The issue seems more severe with any Duty Finder instance. At times, the action on my screen pauses for 20+ seconds! This started on 8/25 or 8/26, but in phase 4 beta everything was smooth as silk.
Playing a healer is pretty much out of the question, and a tank is not much better. Even my damage output as DD is greatly diminished because I'll be hitting mobs that are still alive on my screen that have long since been dead. This is the only game I have issues with. All other online games run great.
I love this game, but the lag is killing it for me because I can't be a reliable player for anything! Knowing Verizon, they won't do jack about it so it's up to SE. That doesn't exactly make me feel great. All we can do is continue to raise awareness on the issue.
well ive done a bunch of trace routes to the server hubs from my northern va fios connection, i saw some packet loss but nothing constant enough to make me believe there is a problem. ive monitored my ping to a google server to see if it was my end of the connection messing up, but it was solid for about 2 hours while the game was lagging. the game is pretty much unplayable for me as a healer right now. its constant freezing still every 10 seconds
i made a character on an EU server and sat in town, i didnt notice it at all there. I'm not sure what the problem is, but i dont really know how to bring this to their attention. they have other stuff to deal with i guess. i just dont know what changed from saturday and sunday, it was playing great. i even made a comment to someone about how i havent experienced lag one time
Hopefully new servers will fix something. <sigh> why didnt they just put the servers in texas or chicago.
same here been lagfest for the last 48 hours. I can not even do fates because of the lag.....and I want my chocoboo. better switch to blm and spam ice :P
Exact same issue and I am here in central Texas, going through Time Warner Cable. What is weird is when I make it to the Inn the connection is perfectly fine (reading the network info on the top right) but has soon has I go anywhere else whether its a main city or some place way out of the way with little traffic, its the same. Its not a router or wireless issue either, I can run Crunchyroll and YouTube just fine AT THE SAME TIME and no issues, but FFXIV on its own while I actually do connect has lag issues outside of the inn.

Funnily enough I barely lag at all from 5am - 10am (GMT+8)

I'm having the same issue dungeons and fates are a nightmare. Like others have stated I also have Verizon Fios which is odd. Everything was running fine until the recent maintenance before that I only ever had a little lag. I hope this is fixed quickly.
If you don't mind me asking, where are you connecting to and from with the VPN?
I'm in Virginia with Fios like most here and am sick of tanking with this throttled bandwidth crap.
From what people are saying the servers are in Montreal?? So if I VPN to Montreal I should be good...
