I use two hands on keyboard, right hand switching from number pad area to mouse when selecting someone, also using the regular arrow buttons for camera while using the number pad.
my mouse scroll key is assigned so that, when clicked, it hits the target of my current target--great for when i need to quick-heal a tank in the middle of DPSing, as I main AST. the two side buttons are assigned to shift and ctrl, so I just hit that with my thumb and whatever key I need on my tartarus keypad. emnity is listed right next to the party list, and focus target (usually the tank) is near the top of the screen next to my current target's hp. I'll post the actual HUD layout when I get home to show where everything is set up, and switch to AST to show my skill layout, but this is what I have on-hand while at work

Basically I wanted a set up to where I would never miss another mechanic ever again, which is why the font size is large enough to be seen from space. I have terrible eyes, so my icons are probably a bit larger than most people would go for.
Mine is the opposite, I love practical, efficient, and gives me everything I want in an instant.
To show this, you need to see the full set up, otherwise, it makes no sense.

Honestly I see some hotbars that would send me into a panic attack if I had to try to play with it. How do you people do it?!
I've kind of been wanting to get into this. The one I am posting is for my RDM, which is only 52. However, it is also the most recent job I am playing. What I have quickly noticed is that I continue to find new ways to improve my UI every time I start playing a new job, and incorporate those improvements through modifying the UIs on all my other jobs (the ones I play frequently anyway).
Another reason why I chose RDM is that despite being low level, it remains a DPS job, and has a more complex UI setup than my healers and DRK who are at max level.
Since you are looking for ways to improve, this will be a kind of psuedo-UI guide of sorts. While I am not saying UIs have to be set up this way, they should always be set up in a way that makes sense, and helps you optimize your performance.

The first thing I will point out is how important I find it to have all the information you need to be easily and readily accessible on your UI without interfering with the information given to you directly on the battlefield such as AoE markers under you. Element size comes into play here. You want to be able to clearly see the information, but also not have it overwhelmingly be in your face. It also should be condensed and located in one location. You don't want your eyes to have to constantly travel from one side of the screen to the other in order to access the information.

I play on controller so the xbar is present at the low center. I don't actually need it for input purposes, and is used more for reference. Those who play on controller have somewhat of a handicap, but let me elaborate:

When setting up the xbar, you can assign skills to the Dpad, and also the 4 face buttons. What is important to know is that skills you assign to the Dpad can and likely will sacrifice movement because you use your left thumb to move but also input those commands, so you want to at least try to not put highly used ogcds or skills designed to give freedom of movement to these keys. Instead you should put GCD skills here, or off globals with really long CDs. This gets a bit more complicated if a job has a lot of instant skills such as melee and physical ranged DPS. The concept remains the same though, and you want to assign skills on your controller in a way that doesn't impede your performance.
Now the xbar is also set up a certain way, and is influenced by the job gauge. RDM for example uses black and white mana. The gauge displays the black mana on the right, and the white mana on the left. So what I did was assign my black mana generators using R2+face buttons, and my white mana generators as L2+face buttons.

To the right of the job gauge is Verfire, that will proc under the condition 'Verfire Ready'. Respectively Verstone and 'Verstone Ready' are to the left of the job gauge. These are my indicators letting me know to use these skills instead of Jolt.

Above the job gauge are my active status effects, good and/or bad. In the case I provided, you will see that Acceleration, Verstone Ready, and Verfire Ready are all active. Dualcast was just spent which you can clearly see on the battlefield. Any other buffs/debuffs will show here as well, and I can clearly see the timers on each. What I am constantly looking for is the Dualcast buff, which tells me to not use Verstone, Verfire, or Jolt. It also tells me I messed up if I see all of them are active. When I see the dualcast buff, I immediately know I need to follow up with Veraero or Verthunder. Everything flows downward from the top.

Above the job and status effect gauges is the focus target bar. This gives me all the information I need about the target without overlapping any of the information beneath it. Including the target's HP%, and status effects it is benefiting/suffering from. As RDM 52, I have no status to give a mob other than death.

Below the job gauge are certain ogcd skills. Centered and laid out horizontally. You might notice that all three here are also on the xbar. The reason this is because they are a bit bigger to facilitate viewing their wind down, but more importantly, they are placed in the order they are used in my openers. As an example, as RDM I will always open up using Acceleration, and after some other skills are used, the next one I use will be Corps, and then Fleche will come following my next dualcasted spell. This will expand as my RDM levels up, and the hotbar is set up in a way to allow this.

Below that are the HP/MP gauges. And their placement here is actually VERY recent. Previously it was in the default position, centered below the xbar. They got moved to right where they should be when I started playing BLM after playing RDM, and I quickly noticed how much my eyes were going from left-center to right to access the information.
Want to know what happened as soon as made this change? My headaches that have been plaguing me for weeks totally went away.

As for the remainder of the UI: To the very left is the chatbox, set to five lines with just a tad of transparency added. Above it are the global passive buffs such as 'Reduced Rates, and also the ogcds that I still need to monitor, but are used under situational circumstances instead of as soon as you can when they become available.

As a general rule of thumb on controller, you want to be able to view your cooldowns that are not on your main xbar at all times. So strategic placement of hotbars and other UI elements is how you accomplish this. I also find it ideal to place the skills you use the most on the default xbar, and the xbars you access using single-press/double-tap of L2 and R2 should have the skills you use less frequently, while still keeping your freedom of movement in mind.

To the upper left is the Party and Enmity lists. If I was in 24-man, the other alliances would show up just underneath the party list. This information becomes more important, the more support that job provides to the party. Needless to say, as a healer, I look here a lot. I see a lot of players placing their party list right in the center of the screen, and this would bring those headaches I was just liberated from right back.
Center top is my exp bar, and boss bar. Both there just for reference and out of the way. And lastly to the top right is the map and duty list, just set to an element size that I can easily see them when I need to.
Anywho, I think that is about all I got for right now, and glad to have this off my chest xD. Anyone who read all this, you rock!

You can't hide the job gauges, so I stick them in simple form down there, along with some other useful out of combat stuff. EXP bar covered by square's watermark in the bottom left there. I also put w/e class abilities I want to access out of combat there like summons, stances, my wrath clear button cus I hate fire swirling around me when I talk to NPCs, or the peleton on DNC. They're all on hotbar 4's first few slots since I always have the room.

Combat UI (HUD slot 2) has all the same toggles available, as well as combat hotbars available. The smaller three can be toggle hidden, but I usually have them up for cooldown tracking. Also, in combat, I actually have keybinds set up so I can switch the active hotbar between hotbars 1-4, so I'm only interacting with the current hotbar. It works for me.

Oh, and my mount icon is a macro to also levelsync when I dismount, cus nothing is more annoying than hopping off at a FATE and attacking while forgetting to press that button.
Just wondering, but does anyone know a way to macro or set a keybind to turn player names on and off without having to go into settings?
To my knowledge there is no macro for this (although it would be nice if I am wrong about this and there is).
The closest thing that I am aware of is to temporarily turn off names by using the hotkey set as "Target Filters" under the "System" tab of the "Keybinds" window. I think it is by default set to the "x" button, so you can press "x" (or whatever your keybind is) to hide character names / click through other characters if you're trying to do something in a congested area (e.g., summoning bell, Ishgard FATE turn-ins, etc.).

Replace company chocobo with w/e mount you want that macro to use. Keep the quotes. And you don't have to include the draught chocobo as the icon, of course.
Levelsync by default turns off when you leave a FATE, so I don't see the point in adding an "On" after it, but I suppose you can. If you just leave it with no on or off after it, though, it'll just toggle from w/e the current state is to the other state. And since its always off once you leave a FATE, and you aren't likely to be mounting and dismounting int he same FATE, then having no "on" isn't a problem to me.

Same with healers - just unsync, heal everyone up, wreck any non-FATE mobs that may have appeared, and then jump back into the fight.
But yeah, anything without the ON or OFF after it generally just toggles it. For example, "/bgm" turns on/off the BGM music, whereas if you enter "/bgm 50" or some other number it sets the BMG volume to that level. Same with a lot of other stuff in the game ("/action error [ON/OFF]". Other helpful macros are "/macrolock" (which prevents other macros from running if you currently have a macro running) and "/macrocancel" (which is great if you use crafting macros).
In terms of layouts, I tried various things but ended up with a single bar at the top that allows you to access various things:

I have buttons that swap me between my normal fishing and spearfishing hotbars on my Fisher, and 5 buttons for my various BLU layouts. For that purpose, it is nice to have 1 button that saves and 1 button that loads BLU layouts:

Ok so during combat, my eyes float a bit, that's why hotbars 1, 2 and 3 are usually split like this. Buffs/Debuffs always at the top. Most single target skills (for any job) on the left and aoe on the right.
As of the rest of the hotbars...yeah, I definitely should be trimming it :p
1-3 changes depending on the job/class. Job gauges for everything in same spot. Guess I could trim out the extra 4 tank gear sets, as I would have one for max gear, and another for lower gear I would soulbind for materia running lower dungeons, but after getting some crafting work in I desynthesis everything, or turn in for Ventures. With Expanded inventory for all. Keyboard and mouse, no controller, and haven't messed with New Game +, so those UI spots are just wherever.
