There should be some punishment for fakers; their behavior ruins the experience of everyone in their party, especially if it's a tank or healer. Balancing punishments for fake DCs against legitimate DCs is always tricky, and this topic comes up a LOT in competitive games (why should I be banned for going to the toilet during a queue etc.). The difference between fakers and legit DCs is that fakers have a behavior problem and will continually do it, and legit DCs should not happen that often (and if they do happen that often, you're effectively no better than the fake-DCers if you or your ISP doesn't fix it).
How some other games handle this:
Street Fighter V: People who ragequit 3 times in 2 hours will be banned for 24 hours and lose points. The system can supposedly discern client-side disconnects (i.e. ragequits) from server-side (i.e. legit DCs).
Overwatch: For casuals, records the percentage of the last 20 games that you left mid-game, client-side disconnects (game can detect server-side disconnects so doesn't penalize), and inactivity, and if it's too high you get hit with 75% XP reduction until that percentage drops enough. For competitive, the rules are more complicated but in addition to XP loss, you can be suspended from games - the suspension time is increased for each infarction until the system decides you're hopeless and bans you outright.
League of Legends: Players who repeatedly leave matches and idle are flagged for a low priority queue and required to play out 5 full matched games to get out (low priority means it's longer than normal queue times + matched against similar leavers). In addition, you are unable to queue for increasing amounts of time. It counts as a loss even if your team won. 1-2 incidents a day isn't enough to trigger this system, supposedly.
DOTA 2: Players who abandon game or get reported repeatedly are placed in low priority queue, and if partied, also causes party to enter low priority. No drops, a certain number of games must be won in order to get out (1, +2 for each abandon while in low priority up to 5, then 24 hour bans) Multiple reports will also toss you back into low priority.
World of Warcraft: If you leave before killing a boss, you get a Dungeon Deserter debuff and can't requeue for PVE for 30 minutes. Vote Kick has no set cooldown, but the more often you personally vote-kick, the longer the cooldown becomes. Vote-kicking applies(?) the Deserter debuff. The idea is that there should be enough reasonable vote kicks to go around to get rid of the people you want to get rid of, but if done habitually you'll be heavily throttled and the cooldown increase degrades very slowly.
I think the problem with FFXIV's system is it's not behavior-based at all and too scared of offending the legitimately/infrequently DCed folks, so the fakers have free run of the system. Either the game needs to be able to detect client-side disconnects (they can't even detect client-side cheating and pos-hacking afaik, and the disconnect recognition is not failproof) or it needs a behavior-based ramping system to deal with fakers.
