Sometimes the game works well sometimes it is very laggy. From my own investigation this is due to many US ISPs including Comcast route their traffic to Montreal, Canada with Level3. The Level3 hop is often overloaded more often at night introducing a lot of lag into Final Fantasy XIV.

The only way this is really going to be fixed for a lot of Comcast and Verizon customers is if you move the servers out of Canada and set up a data center somewhere in the United States. Date/Time: Started around 6 PM EST on December 23th, 2015. Believed it was from people being on from the holidays. However, the problem is still hitting me hard.
Frequency: Every so often. Maybe 30 minutes-hour between the lag spikes. It varies, however. Everything freezes up for 5-10 seconds, then everything speeds forward.
Content: Happens in instance dungeons and the open world. There is no escape from the spikes at all. Being a healer and tanks doing big pulls, these spikes have an effect on me. It can mean between a wipe from mobs or not. It's frustrating.
