The more common of these are when the company gets hacked, if they have lax security and bad methods of password storage. It doesn't matter then how complex your password is. I also remember when adobe flash had a problem that allowed it to put keyloggers on, so there were adds on WoW sites that made you vulnerable. It lasted for like a week before being fixed. So besides being careful, there are still ways to get your account hacked that you can't do much about.
One of the things I have asked SE to do is to unregister my token. I lost my token and have bought a new one, but in order to remove my lost token, I have to use a code generated by that said lost token. Which is not possible, since I DON'T HAVE THE ORIGINAL TOKEN...
But, so far, all I get is:

So, I'm having a back and forward with this.
I don't want to be on the same situation as the OP. I hope SE doesn't kick back my second email.
There is a mobile authenticator, as well as them selling a fob. It has worked flawlessly for me and my account.
They already do this - I couldn't log into my sister in law's account without it stating "suspicious behavior" and locking the account down till you reset the password through an email link and that would then mean someone has access to your emails as well, so you may want to check your PC..
Otherwise, the only way someone can get into your account without resetting it:
1. They already had access via you letting them on and having them reset it, which ties their IP to your account and prevent lock out. I know this first hand because I do it with my family.
2. They know enough about you to reset your info through SE.
Otherwise, it sounds like either someone you trusted or you got logged, there doesn't appear to be a large breech in SE or we'd hear more about it by now.
Good luck getting that token removed. I had that problem in xi years ago when I got it for the mog satchel. Had to photocopy/scan my drivers license and send it to them and provide my entire billing address history.
I don't have a one time password, even though I have 4 un-activated tokens. I have a single SE account, which has 4 XIV legacy accounts and 3 XI accounts on it. These accounts are played by myself, my wife, and my two children. Can you imagine what would happen if you were to pass a token around the house to all those people?
Now, if SE would permit the registering of multiple tokens to a single SE account, say, one per XIV account, then I'd be all for taking advantage of the token system.
