But seriously the whole FFXIV opening movie was very misleading and many people thought of brilliant ideas just from seeing things in the movie sadly none of the ideas or things that were in movie ended up in the game like fire being casted on arrows.

broken limbs would be a good idea but should be healed when hp is restored or it would take to long for the healer to do their job wouldnt it?
but it would add to the immersion of the battles somewhat
but im sure the dev's have better things to do in their time :P

this is a T rated game not an M rated game.
if you want gore go play fall out 3/4. Or wait for the m rated lord of the rings game out. Or skyrim.

There should be fatalities too.
