Could try loggout out and going to Documents\My Games\FINAL FANTASY XIV - A Realm Reborn, find your character's CHAR folder (it will probably have a log folder with recent edits to the logs, which are chatlogs). Simply drag that CHAR folder to the desktop. It will reset your HUD, macros, hotbars and everything in the user interface really.
Login again. If it stops the crashes then one of those user interface settings is corrupt and crashing the game. You can drag each file back in one at a time (overwrite the new HUD with the old one, new macro file with the old one) one at a time until the crashes start again. Restart the game between each change or at least log to the character screen.
If it didn't work, you can restore all the settings at once by dragging the old folder back and allowing it to overwrite. You can make additional backups if you want to in case.
The crashes are too intermittent (15-20 mins) for that process to tell me anything.
If you have no crashes for 5 hours then it probably helped. It is the problem sometimes, although usually it is when the crashes are almost as frequent as right having logged in because that is when a lot of user interface stuff loads.
I was having some intermittent stuttering and freezing, but it was nothing as substantive as a full crash.
I'm having issues since 6.45 launched as well. I have to have Screen Space Ambient Occlusion off or the game runs so choppy that it's not playable. I checked the FPS when this is occurring and it's still at a solid 120. So far SE has not replied to me. It's weird playing with the lighting effects off.
This is happening on 2 computers for me.
Don't know if this might fix it for you but sounds like a similar issue i had once in a while.
What fixed it for me? Just try to check all your .Net Framework (all of the ones one the site here)

I believe I've found the culprit. It appears that the game is throwing a hissy when I alt-tab out of fullscreen. Setting the game to Borderless Window seems to have solved the problem at a small cost to FPS.
