Too often they aren't, but that doesn't matter. Language use is much wider than medical terminology.
Just because a word happens to be used one way by medical professionals in one country doesn't mean it is used the same way in other places, and it most certainly does not mean it is used that way by ordinary people.

In linguistics in general, words are defined by how people are using them, not by how some authority wants to define them.
