A lot of this is that the Internet infrastructure itself is designed to PREVENT hundreds of thousands of people converging on a single data center all at once, because usually when that happens it's a DDoS attack.
The servers my office uses for our application can only handle about 10,000 connections before they choke and die because the VX Rails they're hosted on can't handle it any more. This always causes a problem when the student class sign up period hits, when you have 30,000 undergrads all trying to log in and confirm they got the classes they wanted. The only way to handle it there is to force limit them by class day by day - seniors, atheletes, and honor students go first, then juniors, then sophomores, and finally freshmen.
Square Enix doesn't have that option with XIV - how could they prioritize people fairly? So it's a free for all, and you have to wait until enough people get frustrated and quit to have your chance.

Here's the thing... every MMO, every XPAC we have seen this. SE even announced there would be long queues and errors. They admit they wish they could have upgraded servers prior and had every intention to do so but the "thing" and chip shortage... eh...
