So, I kinda sorta fixed it :D
I've been playing several hours since yesterday, without a crash!
To do that, I've just set the game to run under Windows 7 Compatibility mode.
To do that, right click on the game icon > properties > Compatibility > check the "run this game using compatibility mode" (or whatever the option is in english, that's a litteral translation :D) and choose Windows 7!
Here's hoping that'll fix it for you too, and that the devs will be able to fix whatever causes this later!

I tried running in Compatibility Mode for Windows 7, even not streaming, but the game still crashed...

The error messages you are getting seem to indicate a number of different issues. "nvd3dum.dll" is a graphics driver crash, "xaudio2_7.dll" indicates an audio-related crash and "KERNEL32.dll" sounds like something is interfering with Windows' system files.
What program do you use to stream? Is this program open (or open in the background) while you're playing the game even when not using it? Did this program recently update or is there an update for it?
Your hardware shouldn't have an issue playing the game and streaming the game, but have you made any customizations to the GPU (overclocking, etc.)? Do you have a way to check the temperature of it? Could it be overheating?
I noticed you have two monitors connected. Can you try to disconnect one of them and see if this changes how the game reacts (without streaming).

I use StreamLabs Open Broadcast Software (SLOBS). It is indeed open and running while I am streaming, but it is closed when I am not. I was not streaming today, and the game still crashed. I am not aware of any recent updates to SLOBS.
I am not aware of any customization made to my GPU. I certainly have not been intentionally overclocking it. I unfortunately do not know how to check the temperature of it.
I did try disconnecting a monitor as prescribed. Unfortunately, the game ran incredibly slowly, and after 15 minutes a similar error message with Hex codes following "ffxiv.exe" appeared.

The game appears to have stabilized for the moment. I suspect the recent hotfixes might have helped. I also went back to DX11. I'm not sure if that made a difference, but I think I'm okay for the moment.

I think I discovered what the problem was. For some odd reason, SLOBS (my broadcasting software) somehow reverted a key setting. Instead of encoding my streams with my GPU, it was using my CPU instead.

At any rate, I've fixed the problem on my own, so this thread can be closed and/or deleted.
