My situation is identical to everyone else in terms of my audio setup and the situations the audio lag occurs in.

I testet it and it seems that it happens anytime, anywhere... (trails, old and new 8-player raids, old and new 24-player raids and open world, eureka)
As soon as there are 6 or more players engage in combat, skill audio gets delayed and its getting worse the more players there are.
As far as i noticed, the delay only effects audio, skills work normal..
Turning of other players effects did not help either.

I'm also experiencing this issue. Although anywhere as low as 4-man dungeons can trigger it for myself.

It seems like the only trigger for the glitch is having several sounds playing at the same time.
I've had it occur even in solo situations when involved in FATEs with friendly/enemy NPCs fighting each other.

Having the same issue as well, this is pretty game breaking when you're relying on sound cues for your rotation as well as the boss rotation in raids. I hope this gets fixed soon.

Issue is really incredibly bad, it makes it difficult to play any content due to the audio delay being very jarring.

3) 5.1 surround sound speakers with onboard RealTek HD audio
4) Changed to headphones, to 2 speaker setup and issue still occurs
5) Rebooted PC, changed audio settings and equalizer settings.

I'll throw my 'me too' onto the pile. Happens to me while a large number of players are fighting (eg: Alliance Raids).
Running an Intel Core i7-8700k, Nvidia GeForce GTX 1080 Ti & 16GB RAM.
1) Tried using an external USB soundcard (Scarlett 2i2v2 with ASIO drivers)
2) Tried with onboard sound (Realtek ALC1220 with MS drivers)
3) Tried with graphics settings at Maximum
4) Tried with graphics at lowest aka Standard (Laptop)
5) Network speed is healthy and all other interactions with the FFXIV servers seem fine, it's just audio
Please save me SE!

Thank you for the information everyone.
It looks like this issue has been reported (the post in the bug forums has been moved to 'Accepted Bugs'). Given the possibility that this isn't a bug and you have all hit some weird technical issue, I'd like to give a list of troubleshooting suggestions to see if any of them resolves the issue:
1) If you have any dedicated sound card, try reinstalling or updating the drivers for that device.
2) Check your sound settings in the system tray. Right click it and select 'Playback Devices.' Select your default audio device and click the 'Advanced' tab and choose a lower quality format. See if this has any affect on the issue.
3) Perform a clean boot. Simply open the task manager and select the 'Start up' Tab. Disable everything here and restart the PC. Make sure that no other program is running in the background when you play the game.
If these have no impact on the issue, I'd recommend submitting a bug report. You can do this on the bug report forums (http://forum.square-enix.com/ffxiv/f...2-In-Game-Bugs) or in-game by typing "/supportdesk" in the main chat window.
