Its possible that someone that works at whatever store you bought it from did a nifty job of opening it up, and using the code, then close it back. I've had this happen numerous times especially when I buy online.

Now, I only buy games like this straight from the company that makes it or buy a hard copy from a reputable local store like gamestop where this is much less likely to happen.

Yea, it wass sealed with that PS4 tape on the bottom of the box so surely noone in the store opened it unless they have a way to reseal it back with that ps4 tape... my best guess is that someone, somehow, i dont know how keyloged me and took my code as i entered it in wrong place first and then somehow changed my password not to let me use it before him (my password was normal before entering code)... i bought it from like biggest ps4 store in croatia named Sancta Domenica which are main Croatian PS4 things distributer so I dont think that someone in store used it but who knows... anyway i contacted the store and explained situation and I was gicen a new code that was valid this time
