I had the EXACT same errors regardless of DX11 or DX9 (this was after the susano fight) Is it possible to have a GM move a character out of an area to fix it? As my alts are unaffected

Steps:
1. complete The Pool of Tribute
2. Use the Instance Exit
3. ffxiv.exe errors
4. Unable to access my character
I am also having the same issues and it occurred after completing susano fight. i have seen people say that it is due to DX11 and to try dx9. i however switched to my laptop that has a 1060 vc and it still wont let me back in.
I'm gonna lost my sub days ....
I'm having the same error message after the susano fight. I can't log in at all, I uninstalled and reinstalled game that didint work!
Come on SE! You have to fix that I can't login to my character. This is ridiculous.
Bumping in hopes we get a solution

Game crashed upon leaving the Susano trial. Cannot reconnect, as the game just crashes every time I try to log on with the same exact error.
After complete the Susanoo normal fight and leaving the instance i received the error that others were posting. Multiple log in attempts on my character did not work as i believe its something corrupted on the game side. That is why uninstalling, lowing graphical setting, changing to dx9 and disabling other programs does not seem to solve the issue. I logged in another one of my character and that works fine meaning that it is not something on my end but on Square Enix's side. Hopefully we get a fix for this soon.

Wanted to post here because I'm nowhere near the Susano trial and is not a recurring error.
Just happened to me. Same exact error. Standing next to Raubahn trying to do Cold Steel quest. Had just failed to enter the instance and was typing in chat when the crash happened. Music and SFX still playing but game video completely frozen with error window. Closed and launched game, waited through short login queue and am now fine. Will keep this updated if it happens again.

Win 10 Pro 64
ASRock Z77 Extreme4 MoBo
i5-3570K (no OC)
8GB DDR3 RAM
NVidia GTX 760 with 382.33 WHQL drivers (1 release out of date) connected via HDMI to 1080p monitor (HDMI audio not installed)
