Some combinations I use that I rarely see others use:
- Clemency works with Convalescence and Mantra. Clemency becomes mini-Benediction's at this point. If the groups only Healers are dead you can Sheltron and Riot Blade spam to restore MP and Clemency spam to stay alive (assuming it's not Savage Alexander.) If I'm off-tanking I usually ask the War to tell me when he Clemency's. You can crit heal a Warrior in Defiance with Convalescence up for 20k hp, with a bonus 10k Heal to yourself.
- I usually pop a Vitality potion halfway through Hallowed Ground. This skills invulnerability allows you to get topped off to your new, higher, HP total without worrying about incoming damage. Putting the potion to its full use.
- Bulwark works best when used with Awareness.
- Shield Bash is a very underused Defensive Cooldown. If you've used most of your big cooldowns (Sentinel, Rampart, Hallowed) and are still being pounded on by 6+ trash mobs, give each one of them a 6 second stun. It's a very powerful mitigation tool. No other tank can do this. (Also assuming a WHM hasn't made them Stun immune with Holy spamming.)
Side note: I sometimes see Paladins use Bulwark and then start Stoneskinning themselves. You can't Block and Parry while casting spells.

Rampart works well on majority of cases and has a decent cooldown. I use it as long is active.
Sheltron works best with tank busters you can combine with rampart.
When im low health or in danger i use sentinel with clemency or stoneskin. Last resort Hallowed ground.
About this last, its main pro is that is more useful as a planned tool as it can negate debuffs that deppend of damage. Even ignore entire mechanics.
I use Foresight and awareness together bcz the 2 are weak.
Bulwark is a backup dont use much.

Bulwark and awareness work well together. You can't block crits so eliminating them helps make bullwork more effective.
Shelltron is also a good tool to trigger shield swipes quickly on adds to pacify them giving you enough time to get a full hate combo on them.
A7 is a good example of this. You can pacify the big adds allowing you to get a full combo out and securing hate Before the adds can start casting klutzkrieg or whatever its called the raid wide paralysis. Makes it much easier to stun them.

So just as a FYI using a macro to activate an ability will delay its activation by about a half second or so( enough to get you killed when dropping WD/SG). So unless you really want that marco, or dont mind the delay. Dont use macros for life saving abilities.

For me (if I have my FC-freind as healer) for pulls numbering 5+ mobs: Sentinal (Flashed & Scron cause aoe hate) -> rampart/awarness or Convolence/Forsight -> other of previous selection plus FoF/bloodbath/Scorn -> Rampart if needed cause ant at 4 or less mobs yet or healer needs help. I will try to use the FoF/Bloodbath/Scorn combo after Sentinal if I can but it depends on my mental awarness, mob type & do I got stun a mob attack)
I can agree HG should be uhed as a Def CD but I sujest learning "when" is a good point for it "if" u trust u'r healer/s to not give u that "O crap" moment its usually used for... I use it for "O crap" moment in DF without my friend as healer but if I know a mechanic well enouf I use it to skip it (stun+vulni'd MT = Cover+HG from OT no tank swap required... useable on "Dubblerocket punch" in A8 as OT or MT with comunication) u may even find using it as a "O shit" button might show u a point thats skipable with it by accident.
