Ok i have fixed it finally... Things i did to fix it:
- Upgrade Windows 10 to Anniversary Update edition or later.
- Reinstall/pair the gamepad.
- Calibrate in windows and in-game.
- Enjoy.
PS. The gamepad still needs to upgrade the firmware with an Xbox One console, not only with the Windows 10 Xbox Accessories App.
EDIT: Today i bought the X1 kit play-n-charge and when i connect the gamepad with usb cablem the accessories app recognize the gamepad and let me update the firmware... Only with usb cable.... And the unresponsive issues (1 - 3 seconds the gamepad stuck) gone too :D

I'm experiencing the same problem as Nojh. I have a wireless x-box controller running on a windows 10 desktop though. I checked Skyrim to see if the problem exists, it does not. I have also checked batteries, updated drivers and software, un/reintalled device, all to no avail. It seems to be isolated to FFXIV. I have however noticed that during the 5 or so seconds of control loss, if I move the mouse, control returns. This is the only thing keeping me from walking off platforms or into aoes.

At windows check this controlers hardware properties for energy saving. Disable it. A well known bug of Windows.
