It's not entirely random, it comes from a table of which materia are possible. Like Actus said it's all RNG after that which determines what type and tier from the list you will get.
I can say nearly all of the 45+ belts have a tendency to convert into good Tier IV materia. There is a google doc floating around where people are tracking what they get but you will have to search for it.
