My favourite is still Titan Extreme.
Why: The music and the risk of being sent flying off the platform -It pumps me up every time.
Shiva Extreme and Ultima HM are my second favourites.
My least favourite would probably have to be *thinks* Ramuh Extreme.
I really like the Bismark Ex fight. It plays out a bit different from other fights in the game, and I really like the random aspect of the weather. It makes the fight play out a little bit different each time rather than having to just follow an exact script. I wish more fights had this aspect where you had to actually quickly observe something and change your strategy on the fly based on a random set of mechanics.
My least favorite would have to be Ravana. Ravana just spends way to much time doing nothing and you spend way to much time dealing with scripted mechanics. The entire fight is just a matter of following a script perfectly and I really, really dislike that the fight is mostly position driven.
Any extreme fight where I don't need provoke. :3
I'm very partial to Shiva EX and Bismarck EX for the music, Bismarck EX is pretty fun too.
I really love Titan EX but it is absolutely the most unforgiving fight in the game if you have even the slightest hint of latency.

Best 3:
Shiva Ex - It's a very fun fight, lots of movement, adjustments and very easy to recover from. Shiva also has some wonderful shoes. (Can we have SE?!)
Titan Ex - Great fight to test your reaction time and seeing how far you can get solo tanking. Love the music and once you've remembered most of the fight by heart, made you feel like a god! (Back when it was current content anyway)
Levi Ex - Not to sure why, maybe cause of its hide and go seek nature of Levi's dives? Plus it's the only fight I actually use sounds as a indication of when its time to dodge instead of a visual.
Worse 3:
Ramuh Ex - God no. Having to rely on the orb mechanic was a pain, not to mention folks getting charmed, getting to close to me as tank or when folks forget to clean up orbs..so many deaths.
Ifrit Ex - Disappointing fight. Absolute pain as a healer.
Mog Ex - Having to hold back on the darn things..not to mention Delta..
And the song... THE SONG!! ><

1) Phoenix - Flames of Rebirth/Bennu Phase.
2) Ramuh - The Beard.
3) Bismarck - Breach Blast.
Worst:
1) Leviathan - Too many adds.
2) Ravana - Too much downtime.
3) Shiva - Second half of the fights BGM.

Bismarck ex: like wtf what is with the massive dps check lol

Singlatory reactor: I mean the music alone just sets the mood
Levi ex: that music and it's like a slip and slide lol
Ifrit ex: simply cause it's easy and mindless enough to farm with anyone
