I'm in the same boat as you guys. Got really excited to see it, then worried that I may not be able to purchase the digital version because of this.
This is a PSN issue I'm not sure square can fix it.
I have contact the SE support and they told me to download the trial to see if if PSN unlocks the expansion. But when I try to do this, the PSN told me that I HAVE a PS4 version already purchased. When I told the SE support, they told me to buy a physic version of the game (but, if I do this, will take to long to arrive on my country and I will lose several days of gameplay). So they told me to contact SONY. I did that and the Sony Support told me they will investigate and see what to do. But they also told me if the game was a courtesy, then i cant have the found back on my PSN Wallet. So I will have to wait the Sony answer.

We are aware of the issue where users who participated in the PS3>PS4 upgrade campaign are not able to purchase the Heavensward pre-order for the PS4 on PSN.
We are currently reaching out to Sony regarding this and will update when we have more information.

How will this work for those of us you have our family members under 1 SE account (multiple service accounts)? I see there is a PSN store item for "new service account" when it comes to PS4, will we be able to buy multiple PS4 copies from the store for this?
I also suffer from the same issue of not being able to buy anything but the main game because of the upgrade campaign.

having the same problem here, after a few minutes of the support rep doing "research" he just told me to try again later, well... its later... still having the same problem. I am very very happy that squeenix is on the job but if i end up having to buy it physical im gonna be very very mad. Same situation as our brother a few posts back, its just not gonna arrive on time in my country.
