Afaik, if someone die in extremes, he is doomed to die at any and every mechanics involving party-wide damage. I guess that "sickness" can be safely renamed to the Qarn-ish "doom".
That would make SMN the default member of any static that attempt that content then. The only dps that can Resurrect others.
Out of combat: I agree, weakness is BS.
In combat: no. We already have near unlimited in combat rezzes, there has to be some kind of punishment for gimping and dying.
As far as the thread topic goes: WoW did have some fights with mechanics like these. Azgalor (killed people as a sort of soft enrage), Yogg-Saron (if you get MC'd you need to be killed) spontaneously come to mind.
Personally I dislike these mechanics.

I wouldn't mind it, nothing wrong with variety and different mechanics.
I think the closest thing we could get is a Necromancer boss who uses a spell that causes the party to die, it could be Zombie which makes all healing spells do damage. And then once all party members are dead he'll use his special ability that revives all things dead back from the grave which would summon his minions and revive everyone. Hopefully they could find a way to make it more interesting than what I'm describing though.
