GPU issues.
Try using your on-board/built on GPU instead and take your other GPU(s) out.
If you can not do these tasks yourself, do as advised above: Take it to a repair shop or have a local friend do it for you.
Generally speaking, it sounds like it could be a combination of the GPU/Motherboard GPU slot/Overheating and potentially Memory.
But most of what that screen tells me, is GPU issues. Which could be on the GPU itself, or it could be on the Motherboard slot connecting the GPU.
Overheating is naturally a potential cause to these issues as well.
