Sounds like all of the above. You don't know the fight enough yourself to cover tank mistakes even though to be blunt, you could have. It's usually not the healers fault 100%, as in the tanks not using CDs correctly, but nobody ever notices that. They just notice the tank eating dirt and a healer standing over him. Once you learn the fight it won't matter what a tank does for mountain busters. It isn't just the MB, most times an auto attack follows as well. I can only think of three spots I even time a big cure to cover a tank in this fight; the MB during the ghoal phases (due to knockback, tumelts incoming), the MB at the beginning of Egi phase, and the MB at end of Egi phase right before plumes. So once you learn the fight you can cast before this happens so that when the tank loses 80% of their HP you are instantly recover most of it back. The other MBs I just cure when I can cast next.

Be ready to cure II those mountain busters.
Start casting cure III about right after he starts stomps. Medica is too much MP.
Medica II when the two egi's are up is awesome.
The rest is just being able to dodge. Healing that fight is not too bad at all if everyone else is doing right.

Could also have been a dps issue. Tanks should be swapping when they get 2 stacks of debuff, but having slow dps makes the swap timing weird and sometimes the tank gets to 3 stacks, if they have 3 stacks while MB'd then expect them to be dead if they weren't topped up prior. A good benchmark for this is if the tank gets 3 stacks before titan does his first jump, then the dps is slow.
I haven't played sch for titan ex, but when I play on my whm, I never see the sch use sacred soil for the MB. They put up aldo and I assume they cast lustrate+embrace after MB while I as whm precast cure I/II for the MB, which is enough for tanks that are well geared and have no more than 2 stacks. For group healing, one succor on stomps should be enough even when the whm is gaol'd. As soon as the whm is free they should be using ds+medica I/II or cure III to keep the group up. If dps are dying at stomps then it could be that they aren't getting the whm out of the gaol fast enough, again a dps issue.
Titan ex isn't a healing intensive fight at all, so I would be surprised if ppl dying were ever caused by a healing issue. The only time where it could be considered that is during the egi phase, which is still a breeze compared to other fights such as balancing healing+dodging tank/group during 2nd lazer phase in ultima hm.

Other than adloq before and topping off, you might want to be midcast of a physik as MB is going off so they get that heal ASAP. And lustrate helps too.
Basically, precast heal.

reading everyone's responses I've concluded that it was because of tanks not using CDs and several of our DPSes getting weight of landed to their doom. Glad to know it's not just me, now if I could find a competent party to practice with (´・ω・`)

The thing is tanks shouldn't even need to use their CDs except for bombs/adds. It's probably bad stack management.
You can pretty much heal off a mountain buster-ed tank just simply spamming succor/medica from the stomps and the jails (that you have to do anyway)
And any MB is healed with 2x healer spamming cure/phyick just once at that ilvl.
The thing is tanks should be managing their stacks at 2 (or 3 if the team is pushing a phase change).
the only "healer" part of titan ex is the add phase. That's when you have full blown mayhem, between 2 tanks taking massive damage, you dodging, and with slow dps, a tank going pass 2 stacks (which is where most tank deaths happen).
realistically if everyone is doing as they should, except for add phase, spamming succor all day and snooze through the fight is totally doable.
To be a better titan healer(sch side). Virus every stomp(except the first phase), which always comes after Jail->landslide, and alternate that with sacred soil if virus is down. After add phase it is jail->landslide ->mountain buster ->stomp
Save eye for an eye for OT at adds and save as much lustre as you can for same deal. Everything else is more or less succor with the occasional aqu.
If you are really good, then help dps with heart phase to push titan out of bombs, and save swift for bomb eating or deaths.

just to add. as a fellow healer. i always aim to Stoneskin the tank before Mountain buster hits.
if you memorize the pattern this should not be so hard. it helps to lessen the damage on tanks. then heal
