bad netcode is bad :(
I've had this happen to me just last night while playing MAR and fate grinding in Quarrymill. Lots of mobs -> Overpower spamming -> Ohshi out of TP -> use Invigorate -> still out of tp -> :(
With using Hallowed Ground, the effects dont happen immediately upon use. You have to time it properly, like dodging aoes in Titan HM. Remember the .3 second delay effects some skills as well, not just aoe damage.
Happens all the time to me, i think they are aware of this but never got fixed.
I haven't had it happen to me personally, but I have heard it happen to others.
One thing I've heard is people are pressing the buttons a tiny bit too fast (like, a GCD and oGCD almost at the same time) and it confuses the game...GCD goes off, CD starts on the oGCD but the effect does't go off.
I don't know if that's what's happening or not though, just something I've heard :/
It's not just Hallowed Ground. There have been several time where I've used Aetherflow and not received the charges but the cooldown started. That got a little dicey a couple times. It happens when I try to chain together skills too quickly, like Energy Drain immediately followed by Aetherflow for example. The animation happens, but I do not get the buff. It doesn't happen every time so I chalked it up to lag; two ability signals going through close to the same time, and only one registered.
This doesn't happen to me that often but sometimes if I'm lagging my swiftcast will go into CD and not actually pop. Also, I've had periods of lag where I have to hit lustrate like 5-10 times before it actually goes off. I don't lag often but when I do your hallowed ground better be off CD :P

Happens all the time, with many skills (noticed it with Aetherflow as previously mentionned, but also as a BRD).
Bad netcode really sucks, but I can understand them messing this up.
What I can't understand, for the life of me, is why they insist on keeping animation delays. Can't think of any good reason to.
They confirmed that they're fixing the delay in several high level/instanced areas, I believe it's coming in 2.1 although I'm not certain. They are not going to be fixing it for the general "world" due to stress it would put on the servers. I don't have the link to the post about it so you can either take my word for it or find it yourself :P
No they are not "fixing" anything. They are just increasing frequency of server position check in some endgame instances. Which hopefully will improve (to what extend, we'll see, hopefully enough) aoe-dodging in said instances. Nothing more.
They don't touch netcode nor animation delay.
