Wizardry online does that and more.
If your character dies, that's it, you're dead. you go all the way back to level 1, you lose everything and other players can loot your body (That's if there's anything left)

No thanks to de-leveling. The most annoying thing in FFXI was doing some event, de-level, and be under the required level for your gear and have to do the rest of the event half naked.
Why is there even a need for a massive death penalty? I can't think of any reason, and that is most likely why most MMOs are doing away with them now. Death is a penalty in itself, especially in a timed dungeon run. A full wipe even more so.
Personally, seeing as fatigue was one of the first things abolished after Yoshi-P came on board as the new director/producer of FFXIV, I never see this being added and I am thankful for that.
There is already punishment for dieing like crazy. It's called "Brink of Death" which basically makes you completely ineffective in battle, in which most important battles are timed.
And, unlike in FFXI where intentional wiping is a completely valid tactic, here death of a group means automatic failure of an instance. So the punishment for death is more substantial here than before.
Again, there's no real reason to do it here, at all. Those who are goal-driven value not dieing, especially those with Sanction. (even after getting a consumable sanction item, it still will be very costly to have to constantly replace it.)
Again, there's plenty cost for dieing, people will be much less inclined especially if more gear like Sanction Gear becomes prevalent.
if they added d-level in this game i would quit just as fast as i did xi because i hated it there and would hate it here as well. that was one of the few things i disliked about xi and it was enough for me to quit that game.
Your not serious? "I'm hardcore" how bout you keep track of your own exp and subtract 10,000 everytime you die and go spend 10 minutes getting it back. Death is already punished, if you think repairs aren't severe enough throw away 100k everytime you die, and YouTube yourself doing it or your total soft core casual. Played XI, hated it then, hate it now.

If its in-game that your referring to.. then yes :p, and dying is dying.. you just respawn back at your HP and suffer a slight Dur. Damage to all gear.. Brink of Death only last for so long ( which isn't long at all) and I haven't had Brink of Death for a long time. I really hope for a better " Fear of dying" system because right now really its just spending 5k here and 5k there....Especially in my shoes when I make 500k-1mil a day 5k is nothing.
The fear of dying due to exp loss in order to create a more dramatic experience is a terrible albeit unique mechanic. Speaking strictly about FFXI, I have done raids where I lost 2-3 levels (75-73) Just because the runs would last 2-3 hours. (Namely End-game) Dying is just as much a part of video games as storyline. In most games when you die, you reload your last save (Normal console RPG) or HP (Bioshock). In some games your body drops and you have to go and fetch it. (Diablo, WoW).
The issue with exp loss is that, I feel as if SE is reaching into my pocket and stealing not only my RL money but more of my precious time. That is not what I want to do when playing a game. While I agree the death status should have a greater impact. Currently they have 5min weakness and Duradamage. While it is possible that they could add Skill CD Reset so that it would make fighting that much harder and the need to stay alive that much stronger. In the end you are paying someone to entertain you. Do you really want them to throw pies (and squirt water) in your face? Is that what you consider fun?
