I generally make it a point to give commendations to healers who are active-minded and don't forget about the DPS (i.e. actually uses Medica II during boss battles rather than wait for other people's HP to sink like a stone, uses Stoneskin as often as possible, uses Adloquium/Succor to advantage). All too often, though, I've run across too many healers who are way too slow to keep DPS alive and functioning and seem to have never put Esuna/Leeches on their hotbars.

I'd like a simply "hello" at the start
I'd like to be asked if I can, and want to heal big pulls
i'd like you to wait those 3 seconds for protect to cast
I'd like you turn around and make sure I'm behind you or in line of sight before you run off and grab mobs.
i get 2/3rds of my comms on dragoon anyway, usually falling into the "OMG a dragoon that didn;t suck! have a commendation."
I pretty much always give a commendation to someone. It'll generally be to the tank if I'm a healer, to the healer if I'm a tank, or to the other DPS if I'm DPS. This is what I do if the run is normal and unremarkable.
If my normal commendation target is rude or unpleasant, though, I'll give the commendation to someone else. Additionally, if someone else is exemplary or in some other way goes above and beyond, I'll give them the commendation instead of my usual target.
I'm almost at 900 now. It felt like getting to 500 took forever but getting from 500 to 1000 is going faster. I have been playing healer more and tend to get at least one commendation per run. When I do four man dungeons I usually commend the tank unless they pulled without paying attention to the party composition. For example, pulling as though there are two black mages when the reality is that one dps is a melee with less hp than me; I'll give it to that melee dps for keeping up anyway. But there have only been a few times when I felt like I wasn't getting respect as a healer.
I used to feel this until I started playing other roles. They're all equally important. Tanks get more respect because there's so few of them. That's a community problem.
I'm a healer, and I gain some commendations here and there... But as I normally Q with my MNK fiancée (meaning I normally have 2 options of ppl to commend), I normally find myself commending the person who was NOT a dick during the run. Most of the times, it's the other DPS.
The only thing I can think of is your not doing a good job, because I get commendations all the time as a WHM!
