Don't know if it's just my experience but I did a few tests and when you're zoomed in, it happens a lot more often than when you're zoomed out, especially at events where multiple skills or LBs activate at the same time.
I did 24 rounds of zoomed in and it happened 3 times
I've done over 37 with zoomed out and hasn't happened once.
