I have been having similar troubles inside toto-rak which is suddenly cool again (yay seals!). after dropping magitek things the walls will sometimes persist, other times they drop but when i back-track they're closed again. For me, moving far enough away that they're off of the mini-map then going back gives them the chance to reload and change to their proper "open" state, but it slows me down and is a bit irritating.

1.Killed the jellyifsh that pop the blue circle
2.They had 0 HP, but remained on screen for our whole party.
3. When they finally depoped, the blue circle never appeared.
4. Could not open the door.
