The grayed out ones are actions that were removed with 5.0’s launch. Just remove them from your hotbar. The others that are dimmed (the Summon icons, Aetherflow, Bio II, Miasma) have SCH versions that will need to be manually assigned to your crossbar. To do this, go to the Actions & Traits menu under Character.

Yes, but I doubt they should be assigned to your hotbar by default when you only unlocked the class AFTER 5.0.
