Steps:
Was attempting to use Homepoint #1 to warp to Windurst Woods. Was using Homepoint and I Alt-tabbed from FFXI's windowed mode to another tab while loading/having loaded the homepoint menu. Somehow it got stuck with no menu, no ability to select or exit. Had to hit ctrl-alt-delete which forced a crash of windowed mode to exit. Could not repeat. Tried a few times at Windurst Woods AH and Bastok Mines AH.
I do not think I was hitting any macro keys or anything at the time. Interestingly, there is a chance I may have fat-fingered it and hit my attack macro or last synth at the exact time. Trying to replicate the bug I did manage to cause two failures with my macro on the /lastsynth macro. The menu loaded or the synth went through a majority of the time, but 1/5 attempts I got it to Synth fail or Synth Interrupted still.

While hitting the synth macro and pressing the lastsynth macro I got Synthesis interrupted and once more I got Synthesis failed. You have to hit them at about the same time as you click the Homepoint. It is very finicky, but it can mess with menu load, but it did not lock me into any screen this time. Alt-tabbing or a slow PC may be to blame.
UPDATE: 10/11 - 12:19 AM EST - I got it to trigger again. It was not related to alt-tabbing or anything. I had just exited the Mog House at Bastok Mines and went to the Homepoint at the Moghouse HP (in Bastok Mines), the loading equip and item icons were still active in the top right corner of the window. It was within seconds of exiting the mog house. The first instance occurred shortly after zoning too, I hit the Homepoint crystal within a second or two of it appearing on my screen while the network icons were loading in my equip + items. This may be related to it being a latency thing when all the gear/character models haven't loaded in yet.
Specifically, while this bug happened the item pouch icon was loading, but the equipment one had finished. Bahamut has 2200~ people logged in and I dunno if this has to do with the latency from my end or network side. I also have the Config-Windows-> Multi-Window setting on, and it was in its default mode. I noticed that when this bug happened the top window (displaying /yells) did not appear either, and it had to close/interact with the screen to display the menu. I run a 1920x1080 screen size, so by default more than a third of the screen vertical height was covered by the lines.
Just throwing out observations hoping something useful will lead to this bug being found and fixed. Not sure if anyone else can provide specifics, since this seems like a weird one.
