Steps:
Attempting to complete Campaign Ops Iron Anvil I (W) and when giving the correct recipes for items, it is saying it is incorrect and results in failing the mission. I was given Sturdy Slacks for the item, I chose: Earth Crystal, Lambent Fire Cell, Lambent Water Cell, and Slacks. After selecting these, the npc states that he doesn't have time for me to remember all of my recipes, and the mission fails.
I also failed a 2nd time on Bone Mask with these options: Earth Crystal, Sheep Leather, Bone Chip, Giant Femur.
Both were the correct recipes.

I tried Iron Anvil I (B) and it seems to work OK here.

The "npc states that he doesn't have time for me to remember all of my recipes" bit seems to me like it might be hinting towards running out of time while selecting the materials.

I'd try again doing it faster, if possible, and hope that helps!

I'll give it a shot, but the first one I completed with no issues, only the second one had issues. I'll let you know if it works though!

So, it's very possible that it was a timing thing. I just ran the op again and I got the same item, cleared it with no issues, definitely faster than the first time because I remembered the mats from earlier. I'm going to assume it was a timing issue. I appreciate the response!
