Sortie - A sector - Brown "Chest #A3" occasionally does not produce the "Shard: A"

Area and Coordinates: Outer Ra'Kaznar (U), near A Boss Telepoint.
Party or Solo: Party with 6 members

Monster Name: Abject Leeches
Steps:
1. Kill leeches with Magic Kills to spawn "Chest #A3", "Chest #A4", "Casket #A1".
2. Open chests, starting with Chest A4, then Casket A1, then Chest A3.
3. Opened "Chest #A4" and received "Metal A", opened "Casket #A1", opened "Chest #A3".
4. Proceeded to Telepoint. At this time we all realized Shard A was not received by anyone, due to the inability to enter Dhartok's room.
6. Tried killing more leeches with magic kills to re-pop chest, but as speculated prior, this did not work.

In Sortie tonight, my party completed the objectives in sector A to get the Shard A, Metal A, and Casket #A1 from the Abject Leeches. Upon opening the chests, we did not receive the "Shard A", locking us out of Ghatjot, which in turn locked us out of Dhartok. I know this has happened to my group in the past, months ago, but clearly this issue can still occur. Please fix this!

Note: We did collect the Metal A before Shard A. Maybe this is part of the bug?

Please look into this SE. This has happened to me a few times, I bet it has happened to many others as well.

Did any other box pop at the same time, I have seen an A boss box pop in C area when both objectives were completed at the same time. So perhaps the box did pop, just not in the correct area.
