I think the most important thing right now is just fixing the current PvP content such as job balancing, damage caps, etc. On Fenrir, players still participate in ballista with an average of around 14 members in total. However, many of these matches are very one-sided and it can get pretty frustrating.
Here's just a few of the issues/ideas:
Damage cap
Now that we're lv90, the damage dealt can get ridiculously high. For example, a well-geared Impact can do 1400 damage, Wildfire can deal 1000 damage, Stoneja can deal 900+ AOE.
Job Abilites/Traits
Like Provoke (WAR), which has been reworked specifically for ballista, job abilities like Snarl (BST), Accomplice/Collaborator (THF) and Ventriloquy (PUP) could be reworked accordingly since currently they have no use whatsoever. Moreover, there seem to be job abilities that are too strong. For example, Mana Wall (BLM) which decreases damage taken to 50%. Personally, I'd like Charm (BST) to bind since it binds for 10 secs top and resistance is built incredibly quickly, but I can see how it's a problem with its 10 second recast...one can hope!
Incentives/Rewards
Ballista JSE equipment or a substantial increase in experience points and money you receive at the end of the match would definitely be nice.
I really enjoy PvP in FFXI, I'm just disappointed that such a great system has been totally abandoned.
I had a lot of fun when PvP was first introduced way back when... but it died really quickly and I think for the most part people don't even remember that it's there... I'd really like to see it revitalized...
