Steps:
- I was standing in front of Ghatsad at I-7 as Pup 99, to get my Spiritreaver Head for Puppetmaster.
- First he said he just has to do a few adjustments and he would be ready. Touching him a 2nd time just caused him to talk about fighting styles.
- I did the quest for the Spiritreaver head months ago. I know i traded all the necessary items. Then i forgot about the quest. After a few months i got back to get the head. I tried to trade Ghatsad some Imperial coffee, like ffxiclopedia suggests. But he didn't take it or didn't react to it in any way. Since i did the quest already, Ghatsad didn't give any option to do the quest for a head now.

Addition:
Today Ghatsad just did give me my Spiritreaver head at 23:30 GMT+2. I'm not sure if the quest got extended, cause i didn't check Ghatsad after i gave him all the items. But i don't really care now.
