While fighting adze earlier today we got red, but no key item was rewarded (no deaths, unclaim, already having KI, etc). Is this a bug or am I wrong in thinking red!! is 100%?

It should be 100%, but not sure what else could have gone wrong. Maybe someone else claimed it first? Claim counts, not aggro. Are you absolutely sure it didn't deaggro, and the person who claimed it was not dead or out of zone? Or was the person who claimed it talking to flux or otherwise interacting with an NPC? I'm not even sure if NPC interaction even prevents it, but no idea what else could cause it at this point.

Did you steal the mob from someone else who claimed it first then disengaged to make it go yellow?
There was a Japanese player that would grab NM's and attack the other mobs while they had hate from a NM so people would attempt to grab the NM. Problem was unless the player who claimed first died/zoned they'd still get the KI.
A LS I was in claimed tunga from a player who said it wasn't claimed. She appeared to be doing AOE chest farming. After we killed it the player said thank you for KI and left. She basically used us to kill it for the KI.

It isn't 100%. I have had this happen to me as well, and I have a friend who had the same thing happen as well. Mine happened on a pop nm, so I know noone touched it beforehand.
Just have to get a new one and kill again.

Just like how you need 26% haste in gear to reach the 25% gear haste cap?
If it were 100%, this thread would not exist. Because SE has to deal with binary rules/coding, it is probably something like 1023/1024. There may be some weird results if they set it exactly to 100%, so they set it to something ridiculously close to 100% to try to compensate.
That's a display error, not a math error. You don't need 26% Haste to cap it, but 25%. Only gear doesn't display it correctly.
Of course it would, because players make mistakes or don't fully understand the game dynamics and don't know any better. This could well be one of those examples.

Red proc has never guaranteed key item drops in abyssea. I've only seen it fail a handful of times out of hundreds of successful proc/kills, so it has a very high success rate, that's all. Probably a ~1% chance of failure.
