I've done salvage for 3+ years now and to get the 5th floor frog to pop, 3 members of party must be totally unlocked so that there are no pathos icons appearing on 3 characters. I confirmed with the 3 members, that they were all unlocked, full gear, stats, mp, hp, ja, magic before I warped from 4th floor north porter to 5th floor north. The Poroggo Madame did not appear, this is the first time I've seen this happen in 3+ years. Was the number of people that need unclock increased by accident? Please check on this as I'm still doing it for alex and thoes in my group still want to do it for gear as well.
