Steps:
It's a weird thing to do, but I have done it on both Puppetmaster and Dancer while soloing. I will be fighting 1-4 leeches, and know that I'm not going to be able to beat them. So what I do is I use a Scroll of Warp while Reraise is up, from either a scroll or the Field of Valor books. The warp animation will complete, but I will die before my character leaves the area. My body will disappear and the "reraise" dialogue comes up. My character will be invisible from that point on and I will be unable to see myself at until zoning or something else happens.

I would include screen shots, but the servers are still not up at the moment so I don't have access to play Online.

Hmmmm. Sounds like you mighta hit the "5" key, on your number pad. This switches your camera view from 1st-person to 3rd-person.

When in 1st-person view, you won't see your character.

Try hitting it again to become visible. (I'm not sure if you can toggle this, while KO'd.)

Also, sometimes you may be against a wall or something and the camera angle is just messed up.

That's not what happened. It's plainly obvious whether or not you're in first person view even if your character model isn't showing up.
