I have a level 55, 40, 30, and 49. Started on July 6th and still no beret! In my honest opinion, I don't think I will ever get it. It's terrible on SE part.
It's funny. Everytime I see someone with the beret, I say to me wife "that's the hat I can't get."
Has anyone ever somehow lost or had to have an item restored in game? What was the timeframe on getting that done? I don't see why they couldn't take the list of people they have gathered and just manually toss them a hat? I'm going to retire from complaining about the hat but I will be checking back just to see if they ever resolve it.

I am deactivating today because of the lack of care about this issue. I would give anything for more people to post that they are joining me. My main is so far past 30 that I'm looking for people to help me with the lvl 60 limit break quest, but it's the principal. I don't know if I could cite a more blatant disregard for a consumer base than this if I tried. I would say the recent Diablo III debacle, but even mighty Blizzard caved to consumer demands and is attempting to fix its game. It's sad to me that this thread is based of an apology posted what, almost 2 months ago? Give me a break. There have been a multitude of workarounds proposed in this thread (I say workaround rather than fixes because when I suggested to an SE rep that they delete my key and give me a new one they responded with 'that is not a fix.') The game growth is dying. It will be completely dead after the relaunch of FFXIV, the xpac for WOW, and the "relaunch" of Diablo III with the new patch. The people that have kept their subs alive will continue to do so, but it is obvious SE has no desire to cater to any new players. It's a shame, though, because people were looking at this game as something fresh despite its age. Thanks, SE, for failing yet again to give a sh** about your customers.
Hey just letting you guys and gals know I got mine today. A gm personally came to me in CN and handed it to me. So they are actually doing something about it so patience is key here lol.
A GM just contacted me too, in fact, to gather information about my destriers issue. I told him I already got it, but at least it shows that they are actually doing something.
He wouldn't give me a free relic instead, though :(.
I envy those of you who have your berets already, I was waiting to see if they would fix the dispenser soon before posting anything but if they're manually handing them out then I guess I should mention that I'm also missing my destrier beret.

Contacted today by GM Xophious @ Fenrir. He dropped the hat directly into inventory. Thank you SE for allowing a work around to the problem.
I just got mine from GM Xophious on Cerberus.
I think the moogle is fubar'ed, so if you haven't gotten it make sure your name was on the investigation list and play for a bit so the GMs will get around to giving it to you. I had been playing for a few hours, so don't expect them to be waiting to spring on you right when you log to throw the hat at you.
what do i do to get a gm to send me mine?

The team has been working to contact customers that are still affected and were unable to receive their beret once the fix was put into place. Hey, so what's the story? Do we need to post somewhere to be added to the investigation list or do you know who hasn't gotten one? I haven't gotten mine yet and it's been about a month now.
Just checked both of the character this i have with a code registered to them no hat. So going on a month and a half.
My brother has still not been able to redeem his beret either on his account. We tried redeeming his code almost a month and a half ago and still have not had any luck. What should we do now?
As foretold, the issue is resolved as my trial month expires. At least people are getting their hats now. Would be nice if I knew I had one waiting for me when I decide to re-subscribe but since they're handing them off personally I guess I'll miss out on it...again. Sincerely glad to hear some of you received your hats, though.
Haven't played for a couple of days, but if the mods are finally getting around to giving out hats then I think I better log on and start clocking up some more play time!
Past level 30 on my main job now, but it'll be handy for levelling my other jobs.
To answer my own question in case anyone was wondering, yes, if you do put in the code now you do get the beret. It seems to only be the people who put in the code earlier who had their beret fall down a dark digital sewer that SE is still trying to fish it out of.

Right now, if you are in doubt whether or not you are on our radar, feel free to contact the Support Center via the webpage. Support Center > Final Fantasy XI > Contact .
The Support Center is relaying all of the contacts to the GM Team after they do some verification. You can also reach out to the GM team, but if you are not on our "list" we will need to direct you back to the Support Center.
We hope to get everyone their magical hats very soon.

I, too, was unfortunate enough to enter my reward code during the issues and am not able to receive my Destrier Beret from any Festive Moogle. Tried numerous times over the last several weeks.

So a GM contacted me just now in regards to receiving the Destrier Beret.
But here's the funny part. For some reason he was under the impression that my main character on my main account (the one that's visible when posting) was the one eligible for receiving the item. Which strikes me as odd, because in the 2 posts that I made on this forum, both times I mentioned the name of the character involved (Nyanstix, in this particular case) and 1 message that I sent to the Support Center as advised by Jhanaka.
Both cases I explicitly stated that the character which is required to receive the Beret is Nyanstix on Quetzacoatl.
I mean, I appreciate the fact that I'm finally contacted, even after I received a mail back from the Support Center telling me that they can not provide support for this via Email and that I'd need to call or chat with them. But seriously, how can the wrong character get added to the list?
I also had problems redeeming my beret, I've been away from the game for a while so I haven't have a chance to contact a GM until now. After contacting the Support Center I was told to contact an in-game GM, who told me I could not receive the beret any more because of the order in which I redeemed the code.
The problem is that I redeemed the code on this account in the exact same way that I did on my other account, where I had no problems retrieving the beret since it was before the redemption issues started.
Even though the GM confirmed the code is valid I could not receive the beret. Not really sure where to go from here, any ideas?
