Steps:
1. Kill Karkadann (red & blue !! triggered.)
2. In treasure pool: Orison Locket, Iga Dochugappa, Wieldance Card.
3. Party members lot on items, several people lot on Wieldance Card.
4. After some discussion, all but one party member passes on Wieldance Card.
4a. Party member with remaining lot on Wieldance Card has 64/80 items in inventory, and one other Wieldance Card in Mog Sack (not in inventory.)
4b. All members but one with remaining lot have passed except for one other member who had not lotted at all.
5. Party member who did not lot any treasure leaves Abyssea - Vunkerl and warps to Port Jeuno.
6. Items in treasure pool are distributed. Message in chat log specifies that one party member has received Orison Locket, and that Iga Dochugappa was lost (as all remaining members in zone had passed on it.) No message is displayed in the chat log regarding Wieldance Card.
7. Treasure pool is empty.
8. Party members all check inventory, Wieldance card was not distributed to anyone and simply vanished from the treasure pool. No further items were added to treasure pool, and party members were leaving the zone (other than the one with remaining lot.)
Honestly, this is not the first time I have seen this issue happen. I have no idea what causes it, and I've attempted to provide as much information as possible about this particular instance. The GM call I submitted regarding this issue has not been responded to at all.
Edit w/update: Apparently GM Ashvrei responded while I was asleep.
