I have been wondering this myself, since my disc stopped working the other day (was clean, no scratches, but console kept telling me it was unreadable after like 10+ attempts.) I do not need the codes for a new account. I only need a disc to be able to run the game on my console since for some reason it's not possible to just play from hard drive like on pc (Which would be awesome if it was possible by the way because it would eliminate problems like this unless the hard drive broke.) I have also heard recently though that it's almost impossible to find a copy of FFXI for the xbox360 without just buying it on Ebay for over a hundred bucks. Considering I am already paying Square for two accounts with multiple characters (mostly for storage reasons... I'm a bit of a pack rat I guess...) I must express just how happy and grateful I would be if I found out that they do indeed offer replacement discs. (It also appears to be impossible to uninstall it without a working disc as well if for any reason I wanted to take it off my console, which I don't, but I would really like to be able to log in on my console for when I go out of town etc. and am unable to take my pc with me.)
There is a glitch with xbox discs where sometimes the xbox says its unreadable and wont allow you to do anything. I found if i swop it out for one of my other ffxi expansion discs it reads the disc and plays again. Dont know why it happens but it does. Both discs become readable by doing this.

For the unreadable disc error on xbox you can also try playing an mp3 file from a usb device (a normal thumb drive would work) and as the music is playing boot up the disc and PlayOnline should start up but warn you that some data might be corrupted and will give u the option of proceeding with login ore reinstalling simply proceed to login and things should be in working order.
As for disc replacements I'm not sure if SE does supply them. I doubt it.

Bummer, I only had the one disc (it was an all-in-one thing) and because after 10+ tries of it not working even though it would run other games fine, I figured it was shot so I junked it... what good is disc if console can't read it?

the disc wasn't the problem its the way the console handles the data or accesses it or something but for some reason playing an mp3 file and booting up the disc while it is playing figuratively smacks the 360 around to stop with the BS and read the disc correctly. Sorry to hear you tossed the disc tho.
