My controller drops connection every so often, for 4-5 seconds. This happens no matter:
1)Which controller I use (I own several, all Microsoft PC game controllers)
2)Whether I use it in Bluetooth mode or wired

This does not happen with any other games that I play.
I have read that this is a problem with how both FFXI and FFXIV handle Windows 11 polling for new devices. I uninstalled all my Oculus software (a known cause of frequent device polling) but it still happens. If I leave the Device Manager window open on a second screen I can see this happening: the DM shows the polling happen, and FFXI loses the controller for a few seconds.

If your controller has that option (some gaming devices from razer et. al. may) try lowering the polling rate. I had an issue like this with Battlefield 2042 when it launched where if my mouse's polling rate was at its fastest, the game would bug out like crazy and not respond properly to input. Lowering the polling rate fixed it. Options for this are common for gaming mice and keyboards but I'm not as sure about controllers.

I updated the firmware on the controller, and now it drops out less often, but when it does it is way worse now: it doesn't come back for a long time (1+ minutes), and the keyboard also doesn't work properly: ctrl and alt keys don't pop up menus, but escape and the arrow keys seem to work sporadically.

You know it might possibly be a bad windows update or something. My USB headset has been dropping out randomly. Not specifically with FFXI, but it's an odd coincidence. It only started happening recently.
