I know you ask for the template for bug reports but this one is so black and white it shouldn't be needed, I will reformat my post if truly necessary but anyway.
In Dynamis - Qufim next to the higher level Nightmare Weapons is a constantly dead wyvern, it belongs to the nearby Vanguard Partisan as, when he tried to use Call Wyvern, the 1hr animation goes off but no Wyvern is summoned.
