Retrieve an old account?

I quit playing in 2006 and no longer have any of the log in info for my old account (play online ID/password and square enix ID/PW). All I have is the character name, main job, server, an estimate on when the account was created and 1 registration key from an expansion pack. Is there any possibility of retrieving the account?

problem with this form is that it asks for payment method and it's a required field...the card used at the time no longer exists and I don't have that information >.<

You're probably best off finding their phone number and calling in, then.

If you're in the US and you haven't done this in the past 12 months, you can try annualcreditreport.com and find information about your old account there. Or, if you remember the issuing bank, you can try calling them directly and see about getting your old account number from them.
