So, what's up with his spawn/respawn? I have had a character in his spawn room for 132 hours now, and checked as the Troll Mercenaries have been preparing for every besieged (so far two of them) and he has not spawned at all. Am I missing some spawn conditions missing or is he just the longest respawn in the game?

The time span isn't confirmed but from my experience it has to be a lvl 5 or above besieged. lvl 8 has a better chance. But if a besieged is happening he won't be up. Also if he is killed in besieged he won't spawn for about a week. If he is killed in his room he will pop around 24 hours later.
All the advice I can tell you is to just keep the character in the room until you see him. I know it sucks to camp these but that really is the only way to be sure it is up and when he is up.
Wiki doesn't explain it very well on his page. Here is Medusa's page it somewhat explains how all 3 spawn. I hope this helps a bit. GL!
