I had this issue in the past, and I can't quite remember how to exactly fix this issue.
I downloaded the test server once again, and I had the old files from:
http://wdl.square-enix.com/ffxi/preview/US/FFXITC.zip
http://wdl.square-enix.com/ffxi/preview/US/FFXITC2.zip
http://wdl.square-enix.com/ffxi/preview/US/FFXITC3.zip
They were still in my C:\Program Files\PlayOnline\SquareEnix\FINAL FANTASY XI Test Client folders.
How can I fix this issue?

Just to confirm: You are playing from the US, and not the EU?
The files/installations are not interchangeable.
If this is not the case, try uninstall, and before you do anything else, read the readme-file and follow it precisely as the order in which you do things listed there is of great importance.
If you skip anything or, for example, update the client before copying the downloaded files over to the install, bad things will happen.
Just some quick thoughts~

The only thing that can cause this crash is the files from the archives were not copied correctly, or you are using the wrong region files on the wrong region client.

i am having this poroblem as well, i just recently reinstalled windows, installed ffxi and updated etc then i installed the test server as well. the steps taken are as follows
extracted and installed EU client
Extract zip files FFXITC, FFXITC2, FFXITC3
copied the contents of the folder titled ffxi to FINAL FANTASY XI Test Client
run config
login and click "log in to test server" button
wait 2 hours for update
after all that my client just crashes, i am using the correct files i use EU client for public server and EU files for test server.
i just cannot think why its happening because i have been on the test server before, nothing has changed in my setup/software etc. i know people can get on but just unsure why i am starting to get this error.
i have uninstalled and reinstalled the test server 9 times now and run the update same amount of times taking 2 hours a time.
just to confirm, the client that i am installed is from the zip file titled U11204W_001 and using the game files ftom the following locations
http://wdl.square-enix.com/ffxi/preview/EU/FFXITC.zip
http://wdl.square-enix.com/ffxi/preview/EU/FFXITC2.zip
http://wdl.square-enix.com/ffxi/preview/EU/FFXITC3.zip
i am totally confused now

That'd be wonderful except this doesn't resolve the issue for win xp users like myself.
