Logging to ffxi it crashes!

first i had the issue where i wouldn't be able to logging to play online cause my square enix screen on my 360 would crash. so i reinstalled it on 360 and now i am able to proceed as normal, now my issue is with the logging to the game itself. what it shows is the error code POL 12321. i don't know what that means, checked online if something went wrong or if i did not put the info right. and if the issue is from my side then it something with the 360 since i begun having this issues only after i change to another 360. please let me know soon.
